Complete Client Intake Package

All new consumers must complete all three: Intake Application Package, Client Intake Assessment, and Functional Assessment. Please complete all sections of each form. All incomplete forms will result in delayed paperwork processing. If you have any questions, please contact the front office.

Psychiatric Services of Carolinas Patient Registration Form

This consumer/patient registration information form is for those that require or need treatment with our psychiatric team for an evaluation and/or psychiatric services. This intake package must be completed prior to any psychiatric service.

Consent forms Only

Only complete these document if you are required to update annual consents or submit for the first time consent forms. No need to complete these if you have completed the above intake package unless instructed by an office staff member. These documents are included in the above intake application package.
